SECTION 1307.21 OF 21 CFR REQUIRES
THAT ANY REGISTRANT DESIRING TO VOLUNTARILY DISPOSE OF CONTROLLED
SUBSTANCES SHALL LIST THESE CONTROLL SUBSTANCES ON DEA FORM 41 AND
SUBMIT THREE COPIES TO THE NEAREST OFFIC OF THE DRUG ENFORCEMENT
ADMINISTRATION IN THEIR AREA. THE DEA 41 IS T PRIMARY FORM USED TO
ACCOUNT FOR SURRENDERED/DESTROYED CONTROLLED SUBSTANCES. ITS USE IS
MANDATORY.

1320.8(b)(3).
