THE INFORMATION IS NEEDED TO ASSIST
RECIPIENTS OF FEDERAL FINANCIAL ASSISTANCE IN ACHIEVING PROGRAM
ACCESSIBILITY FOR HANDICAPPED INDIVIDUALS, IN CASES WHERE
ALTERATION OR MODIFICATION OF EXISTING FACILITIES IS NECESSARY TO
ENSURE PROGRAM ACCESSIBILITY. THE INFORMATI WILL BE USED BY
RECIPIENTS, THE VA AND THE PUBLIC IN ASSESSING COMPLIANCE WITH 29
U.S.C. 794.
