Abstract: THE FORM WAS DEVELOPED FOR USE BY LOCAL GOVERNMENTS IN CONJUCTION WITH THE COMMUNITY DISASTER LOAN PROGRAM (SECTION 414, PL 93-288). THE FORM IS UTILIZED TO REQUEST CANCELLATION OF A COMMUNITY DISASTER LOAN IN ACCORDANCE WITH THE PROVISIONS OF THE LAW AND REGULATIONS PERTAINING TO COMMUNITY DISASTER LOANS.
