Abstract: IT IS USED WHEN A PERSON WHOSE PASSPORT WAS LIMITED (BECAUSE OF INSUFFICIENT CITIZENSHIP EVIDENCE, FOR EXAMPLE) ASKS THAT IT BE EXTENDED. IT IS ALSO USED WHEN A PERSON WANTS THE PASSPORT AMENDED (TO REFLECT A NEW NAME, FOR EXAMPLE). THE RECORD OF THESE SERVICES BECOMES PART OF THE PERSON'S PERMANENT PASSPORT FILE IN THE STATE DEPARTMENT.
