THIS FORM IS JUDICIAL IN NATURE AND DESIGNED TO AFFORD APPELLANTS A "PIECE OF PAPER" UPON WHICH TO WRITE THEIR ARGUMENTS FOR CONSIDERATION BY THE APPEALS BOARD.
