This information
collection activity is cleared for six months. When the information
collection activity is resubmitted for continuation it should be in
conformance with OMB Circulars A-102 and A-110 and the Endowment
should justify any elements of information that are not consistent
with these Circulars. The Endowment should also justif formats
different from those prescribed by Cir. A-102 and A-110 and apprise
OMB of the consequences to the Endowments program of strictly
adhereing to the A-102 and A-110 requirements. The Endowment should
no use these grant application forms for FY83 applicants until
official OMB approval has been received.

REQUESTED INFORMATION IS NEEDED TO
ENABLE THE ENDOWMENT TO DETERMINE WHETER APPLICANTS MEET
ELIGIBILITY REQUIREMENTS AND CRITERIA FOR FUNDING, TO ENABLE THE
ENDOWMENT TO DETERMINE WHETHER APPLICANTS SHOULD BE FUNDED ON A
MULTI-YEAR BASIS, AND TO PROVIDE BASIC INFORMATION ABOUT STATE ARTS
AGENCIES NECESSARY FOR THE NORMAL FUNCTIONING OF THE
ENDOWMENT.
