EXTENDED PAYMENT ELECTION. THE EXECUTOR OF A ESTATE CERTIFIES THE
STATUS BY SIGNING THE LETTER. SECTION 20.6166(F) (II) OF THE CODE
OF FEDERAL REGULATIONS REQUIRIES THE EXECUTOR TO PROVIDE THIS. THE
DATA IS USED TO DETERMINE WHETHER THE ELECTION SHOULD BE
TERMINATED.
