ADDITIONALLY, USE OF THE FORM SAVES THE TIME AND PAPERWORK FROM FILLING OUT ALL THE SCREENING OVER AGAIN. IT IS USED ONLY WHEN THE SCREENER CAN PROVIDE SUBSTANTIVE EVIDENCE TO DEMONSTRATE THAT THE APPLICANT HAS MOTIVATION AND POTENTIAL TO COMPLETE THE PROGRAM.
