Abstract: THIS FORM IS USED TO CLAIM EXEMPTION FROM THE WINDFALL PROFIT TAX UNDER IRC SECTION 4994; TO CERTIFY QUALIFICATION AS AN INDEPENDENT PRODUCER UNDER SECTION 4992; TO MAKE THE JOINT OPERATOR ELECTION BETWEEN PURCHASER AND OPERATOR; ETC.
