Abstract: FORM IS USED TO SUPPORT A DISTILLED SPIRITS PLANTS BOTTLING OPERATIONAL LOSSES OF TAX DETERMINED SPIRITS RECTIFIED AND/OR BOTTLED. FORM IDENTIFIES THE DISTILLED SPIRITS PLANT TRANSACTIONS ABOUT SPIRITS ENTERED INTO THE BOTTLING PREMISES BY QUANTITY AND SOURCE, AND OTHER TRANACTIONS TO DETERMINE AMOUNT TO CLAIMED. FORM IS USED BY ATF TO VERIFY THE ACCURACY OF THE CLAIM FOR TAXES.
