PAPER AND TUBES ARE REQUIRED TO INVENTORY THESE PRODUCTS. THIS FORM
IS USED TO RECORD THE RESULTS OF THAT INVENTORY. IT IS USED TO
RECORD THE RESUTLS OF THE INVENTORY AND IS FILED WITH THE
BUREAU.
