THE FORM DESCRIBES THE LICENSEE, THE SIZE OF PACKAGES, QUANTITY AND TYPE OF SDS FILLED INTO EACH PACKAGE, THE PACKAGE ID NUMBER, AND DATE OF PACKAGING.
