Abstract: THIS QUESTIONNAIRE WILL BE USED TO ASSESS THE PAST HEALTH OF FORMER AIR FORCE PERSONNEL, SINCE LEAVING VIETNAM, WHO SPRAYED HERBICIDE ORANGE DURING OPERATION RANCH HAND. THIS STUDY HAS BEEN DIRECTED BY THE WHITE HOUSE. THE DATA FROM THE QUESTIONNAIRE CONSTITUTES A SIGNIFICANT PORTION OF A LARGER EPIDEMIOLOGY STUDY. THE DATA DERIVED WILL BE STATISTICALLY EVALUATED FOR EVIDENCE OF VARIOUS DISEASE STATES
