THE FORM PROVIDES INFORMATION FROM
BLOOD DONORS WHICH IS ENTERED INTO A COMPUTER WITH TISSUE TYPING
AND OTHER INFORMATION CONCERNING GENETIC CHARACTERISTICS. THE
RESULTS WILL DETERMINE THE TYPE OF TESTS FOR WHICH THE BLOOD CELLS
WILL BE USED.
