BENEFITS DEPENDS ON SOCIAL SECURITY COVERAGE BASED ON FARM
SELF-EMPOYMENT. THIS FORM IS USED TO DETERMINE WHETHER THE
CLAIMANT'S FARM OPERATIONS ACTUALLY CONSTITUTES A TRADE OR
BUSINESS; WHETHER THE CLAIMANT OPERATES THE FARM HIMSELF AS EITHER
AN INDIVIDUAL OR PARTNER; AND WHETHER THE REPORTED INCOME INCLUDES
RENTAL OR OTHER INCOME THAT IS SPECIFICALLY EXCLUDED FROM
